Section 4: Safety
What general contractors ask for, and the file you send back
Prequalification forms from general contractors, owners and platforms like ISNetworld and Avetta ask small subs for the same handful of documents. The kit has one for each.
| The prequal form asks | You send |
|---|---|
| A written safety and health program | Written Safety Program, edited with your company name and signed |
| OSHA 300A summaries for the last three years | The 300A tab of the OSHA Recordkeeping Workbook, one per year |
| Your TRIR and DART rates | Calculated automatically in the workbook |
| Your experience mod (EMR) | Your mod letter, plus the one-page explainer if it's above 1.00 |
| Return-to-work or light-duty program | Return-to-Work Program, signed |
| Proof of safety training | Toolbox talks with signed attendance sheets |
The return-to-work program is where the money is
Your workers' comp premium is adjusted by your experience mod, which looks back at about three years of claims. Claims where someone misses work count far more heavily than medical-only claims. A mod over 1.00 raises your premium and can keep you off a GC's bid list.
- Keep people workingAn injured employee on light duty, on your payroll, usually isn't drawing wage-replacement benefits for those days.
- Close claims soonerEmployees who stay connected to the job recover and come back faster. Smaller, closed claims weigh less in your mod.
- Show it on paperCarriers and GCs ask whether you have a program. The kit gives you the policy, the doctor's work-status form, the offer letter and a light-duty task list for trades work.

Questions
Who is this for?
Contractors and trades businesses with roughly 10 to 100 employees: roofing, framing, siding, HVAC, electrical, plumbing, landscaping and general contractors who need to qualify for work with larger GCs or owners.
Will this get me approved by every general contractor?
No kit can promise that. Each GC sets its own requirements, and your injury rates and experience mod are what they are. The kit gives you the documents most prequalification forms ask for, so missing paperwork isn't the reason you get passed over.
Will it lower my workers' comp premium?
Your mod is based on your actual claims history, so no document lowers it by itself. A return-to-work program that you actually use helps keep injuries from turning into lost-time claims, which are the claims that weigh most heavily in your mod.
Does it work in my state?
The OSHA recordkeeping and construction rules are federal. States that run their own OSHA plans can have extra requirements, and workers' comp rules differ by state. Each file notes where to check your state's rules.
Is this legal advice?
No. These are templates written by an insurance professional. Have your carrier, agent or attorney review your program before you adopt it.
Can I edit the files?
Yes. Everything opens in Microsoft Word and Excel, and works in Google Docs and Sheets.
